The Least Aggressive ALPPS Variant: Partial-Tourniquet-ALPPS

نویسندگان

چکیده

Introduction: The first variant reported was Tourniquet-ALPPS (T-ALPPS), which occludes the intrahepatic circulation by means of a tourniquet around liver. Methods: We present modification our surgical technique, T-ALPPS, named “partial Tourniquet-ALPPS” (pT-ALPPS) avoids pass hanging maneuver and right extraglissionan approach. Stage this is easier, quicker less risky compared to ALPPS, p-ALPPS or T- ALPPS. Results: In HBP unit 65 T-ALPPS have been performed. Since February 2017 we underwent prospective study comparing with pT-ALPPS, including 6 patients colorectal liver metastases (CRLM) in each group. case performed patient bilobar one them located segment, front inferior vena cava thus could not be passed maneuver. After clearing future remnant (FLR) CRLM, ligate portal vein. Considering that ventral portion pedicle non-vascularized area, aid intraoperative ultrasonography, an Adson clamp through parenchyma. This emerged between middle hepatic A then knotted under ultrasound control. 2, used as for hepatectomy trisectionectomy depending on its location. Conclusion: selected P-T-ALPPS accessible technique easier than passes avascular area.

Two cases of ALPPS procedure: simultaneous ALPPS and colorectal resection and ALPPS procedure for hepatic malignancy larger than 15 centimeter

Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) has recently been introduced as a new surgical technique to increase future liver remnant in patients with marginal liver volume contemplating major liver resection. We herein present two cases. Case 1: a 68-year-old male patient with colorectal liver metastasis was referred to our department. The future liver r...

Rescue ALPPS: Intraoperative Conversion to ALPPS during Synchronous Resection of Rectal Cancer and Liver Metastasis

Future liver remnant (FLR) is the most important deciding factor in planning for liver resection. Portal vein embolization (PVE) was first introduced in the 1980s to induce liver hypertrophy, enabling removal of multiple/bilobar tumors. PVE was later combined with sequential hepatectomies with the aim of allowing the liver remnant to hypertrophy (15-20%) between procedures. However, the interva...

Early survival and safety of ALPPS: first report of the International ALPPS Registry.

OBJECTIVES To assess safety and outcomes of the novel 2-stage hepatectomy, Associating Liver Partition and Portal Vein Ligation for Staged Hepatectomy (ALPPS), using an international registry. BACKGROUND ALPPS induces accelerated growth of small future liver remnants (FLR) to allow curative resection of liver tumors. There is concern about safety based on reports of higher morbidity and morta...

ALPPS: past, present and future.

Complete tumor resection in the liver is the only chance to obtain long-term survival in patients with hepatic tumor or metastasis from other primary cancers. In patients with a large load of tumor within the liver, multiple strategies have been employed to improve resection, especially when a small liver remnant is expected. Staged hepatectomies, in which the surgeon perform partial resection ...
